Adult Community Case Management
The ACCM program is available to those 18 years or older with a diagnosed intellectual disability, autism or other developmental disability. Case Managers help individuals access employment, education, housing, social and other supports, and necessary medical services. They work with individuals, families, service providers, and other agencies to create a Person Centered Plan (PCP) based on a comprehensive assessment of each individual’s needs, strengths and barriers. In addition, Case Managers assist individuals, parents or guardians in navigating the service system, monitoring the delivery of services, and tracking goal progress.
If you have a young adult transitioning into adult services, the ACCM program also offers free information and consultations for individuals and their families who are working to get through the state eligibility determination process.

Representative Payee Program
The Representative Payee program at CCM-4U is provided at no cost for anyone receiving case management services with our agency. In line with Social Security guidelines, our case managers work with beneficiaries and/or guardians to evaluate financial needs and create a budget to submit to our rep payee.
We work to ensure basic needs are met and assist in improving daily living conditions such as major health related expenses, schooling, recreation and special purchases as funds allow. We do our best to keep the beneficiary and/or guardian informed of any changes, remaining funds and general knowledge on how funds are spent. Together, as a team, we do our best to provide a Representative Payee service that is focused on providing the best we can for each of our appointed beneficiaries.
